Wisconsin Medicine is a partnership between UW Health, The University of Wisconsin School of Medicine and Public Health, and the University of Wisconsin Foundation and Alumni Association. All donations to Wisconsin Medicine will be processed and receipted by the University of Wisconsin Foundation, 1848 University Avenue, Madison, WI 53726, EIN: 39-0743975.

Terms and Conditions

The Wisconsin Foundation and Alumni Association (“WFAA”) provides this website and associated services (together, the “Service”) to you subject these Terms and Conditions, as amended from time to time (“T&C”). You agree to use the Service only as permitted herein. The Service is intended only for your personal non-commercial use. You acknowledge that WFAA may update the T&C from time to time without notice, and you accept sole responsibility for familiarizing yourself with, and abiding by, the T&C and any such updates. Refund Policy

Charitable gifts are generally not refundable. Refunds will be made in the event of a mistake or if the purpose for which the gift is given cannot be fulfilled by the Foundation. Individuals who receive refunds have the responsibility of correcting any tax filings they have made reflecting the refunded amount.
